[pgpool-hackers: 3652] Proposal: Add log_disconnections parameter

Hi all,

I implement new parameter of log_disconnections. I have attached this
patch for this email.

If this parameter set to on, client session terminations to be logged.
e.g.
 LOG: frontend connection closed
 DETAIL: connected host=[local] 

Curerntly pgpool can use only log_connections. This new parameter can
make pgpool.log more convenience when we analyze pgpool trouble.
