[Pgpool-hackers] New patches for pcp_promote_node

Hi,

Here is a new patch to replace previous one that concern promoting any
node as new master:

    - Fix preferred node initialization that breaks load balancing after
    promoting.
    - Limit the usage of pcp_promote_node to master/slave and streaming
    replication mode.
    - Add control over node number to prevent node out of range
    (NodeIdOutOfRange).
    - Add a patch for documentation.


Here is debug of error message when trying to promote a node when pgpool
is not in S/R:

    DEBUG: send: tos="R", len=46

    DEBUG: recv: tos="e", len=30, data=NotInStreamingReplication
    DEBUG: command failed. reason=NotInStreamingReplication
