Hi,
PgpoolAdmin confirms if the file exists in $HOME of the Apache user.
It uses PCP commands, and pgpool-II has changed to need ".pcppass" file for
PCP auhentication since version 3.5.

If you upgraded pgpoolAdmin from the previous version, could you try
to install pgpoolAdmin again in http://localhost/path_to_pgpoolAdmin/install ?
