[pgpool-general: 465] Can not get node info

Hello,
I have followed the tutorial at http://www.pgpool.net/pgpool-web/contrib_docs/simple_sr_setting2_3.1/index.html to a t, but can not get the node info to show up.  I have postgres 9.1.3 installed, pgpool 3.1.3 running on centos x86_64 version 6.2 with the latest patches. 
Any help would be greatly appreciated.
